Fight the Flu with Sage Memorial Hospital!
Join us for our Mass Flu Event this fall and guard against the flu. This essential community event is open to everyone aged six months and older. Don’t miss out on this chance to protect yourself and your family.
Event Details:
- Student Day: Thursday, October 19, 2023 – Friday, October 20, 2023 (8 AM – 4 PM)
- Community Day: Saturday, October 21, 2023 (7 AM – 7 PM)
- Location: Sage Memorial Hospital Wellness Center (Gym), US 191 & SR 264, Ganado, AZ 86505
Minors must be accompanied by a parent or legal guardian and don’t forget to bring your valid supporting documents. Registration forms available online now at www.sagememorial.com/flu to speed up the process on the day.

Please follow the steps below to complete your registration form for the Mass Flu Event:
- We have two forms, one for Students (K-12) and one for Community members. Please select the form that corresponds to your status.
- Provide all necessary information on the registration form.
- Submit your completed form either in-person at Sage Memorial Hospital Wellness Center (Gym) on the event date.
- Event dates are
- Student Day: October 19 and 20, 2023, from 8am-4pm
- Community Day: October 21, 2023, from 7am-7pm.
Consent Form(s)
- Read the consent form carefully and understand the information provided before signing.
- For minors, a parent or legal guardian is required to sign the consent form along with the minor’s signature..
- Both the 2023-2024 Influenza Consent Form and General Consent Form must be completed.
- Submit your completed form in-person at Sage Memorial Hospital Wellness Center (Gym) on the event date.
